What form of nomination of a QPPV or a contact person for pharmacovigilance issues is accepted?

It must be clear from the nomination that the marketing authorisation holder is aware that the above-mentioned functions reported to SÚKL have been delegated to a specific person. The nomination should be submitted on behalf of marketing authorisation holder by an authorized person or by the QPPV. According to GVP Module II, the responsibilities of the contact person must be listed in the PSMF.
